(TAN): United States’ Utah state has issued an order requiring all adults to fill out a travel declaration form when entering the state.
Utah Governor Gary Herbert wrote on Twitter that all individuals above 18 years of age who enter the state by vehicle or through the Salt Lake City International Airport will have to complete the form related to the coronavirus pandemic.

“I am issuing an order this week in advance of Easter weekend requiring all individuals who are 18 years or older to complete a COVID-19 travel declaration form when entering our state. This will be required at the airport, and on our roadways,” he said.
The new order, that will be effective from April 10, will ask travellers if they have been tested for COVID-19 and if they have any symptoms, and where they have recently been to.
“The order will go into effect on Friday. The travel declaration will ask individuals if they have been tested for COVID-19 and if they are experiencing any symptoms of coronavirus. It also asks where they have recently been traveling,” Governor Herbert added.
According to reports, people driving into Utah will get a text alert on their phones which will direct them to declare their status relating to COVID-19. The checkpoints include Salt Lake City International Airport, interstates 80, 15, 70, and 84, and US 191, 89 and 40 at state borders, reports said.
“Each person 18 years of age and older entering Utah must submit an electronic State of Utah Travel Declaration Form, as ordered by the Governor, to inform individuals of Utah’s current COVID-19 related restrictions and declare information that will be used by the Utah Department of Health to help track and trace COVID-19 infections that may arise from persons who enter the state of Utah from national or international travel,” the website where the State of Utah Travel Declaration Form will be available said.
